2. Take a screenshot or save your receipt/proof of purchase as an image and upload it by clicking the "Upload Your Receipt - Browse" button in the form. Accepted file types include: JPG, JPEG, TIF, TIFF, BMP, GIF, PDF, PNG, DOC, and DOCX.
3. Fill out the rest of the form. Be sure to double check that your mailing address and email address are correct!
4. Once your receipt has been attached and you've completed the form, click "Sign Up" and you're all set! Prizes will be mailed after this promotion has ended.
